We appreciate the trust you have placed in us over the years. As Myers Sports Medicine & Orthopaedic Center transitions, we want to make sure you have easy access to your medical records and all the information you need for continued care.
Your medical records are available through the PBOmd Patient Portal. Please follow the instructions below.
Important Dates
- Last day of in-person patient care: July 15, 2026
- Office location closes: July 31, 2026
- Telehealth consultations available through: August 31, 2026
Admin staff remain available part-time after the office closes to assist with records transfers and administrative requests.
Step 1 — Request Your Portal Credentials
To request your unique PBOmd Patient Portal login information, download and complete the Release of Records Request Form below. Return the completed form to Myers Sports Medicine by fax at 404-350-9405 or by email at management@myerssportsmedicine.com.
Step 2 — Access Your Records via the Patient Portal
Once you have received your portal credentials, your medical records and electronic X-ray images are available through the PBOmd Patient Portal.
Once you are logged in, you can click individual visit dates to view your records and electronic X-ray images. You may save or print your records and images to share with another provider.
